Hours: 8:30 – 5 pm, Monday to Friday (job share position 2 days per week and holiday/shift coverage).

As part of the professional team, the Primary Care Nurse plays an essential role in providing quality patient and family centered care in an accurate, efficient, and friendly manner.  This position will perform a variety of administrative and clinical duties and responsibilities (but not limited to):

  • Completes a variety of tasks requested by physician – relaying results to patients, assistance during visits if, ascertaining of vitals and other health assessments, foot care, MMSEs, suture removal, dressing changes, etc.
  • Maintaining adherence of patient care plans, and contributing to the implementation of patients’ plan of care
  • Administration of injections
  • Counsels and educates patients on procedures, interventions, symptom management, health maintenance, etc.
  • Answers patient calls and inquiries regarding results and health concerns
  • Obtains a history of the presenting health issue to assist physicians’ pre-op, anesthetic, prenatal, and physical visits.
  • Performs health assessments.
  • Monitors physicians’ lab results while they are away and takes appropriate action as required.
  • Use of Electronic Medical Records for documentation – documents accurate and pertinent patient information
  • Prepares necessary requisitions to order appropriate tests on behalf of the physician within scope of practice.
  • Collaborates with physicians, patients, and other interdisciplinary teams to develop, implement, and evaluate health promotion, chronic & acute disease management, and illness prevention programs & strategies
  • Works as a navigator to ensure patient feels confident throughout process
  • Medication review with patients
  • Other duties as assigned.

Education (Degree/Diploma/Certificate)
• Graduate of an approved Registered or LPN Nursing education program required.

Qualifications and Skills:

• Current Manitoba Nursing license
• Personal Health Information Act (PHIA); Knowledge of Professional Standards of Practice, and Code of Ethics
• Demonstrated effective oral and written communication skills.
• Demonstrated critical thinking/problem solving skills.
• Ability to prioritize care for an individual patient or group of patients.
• Ability to respond to a variety of simultaneous demands.
• Ability to recognize and pursue self-development opportunities.
• Ability to perform independently and as a member of the healthcare team.
• Demonstrated ability to foster a collaborative interdisciplinary environment that supports quality services, quality improvement and staff empowerment
• Proficient with computers, software programs, and applications such as Windows and Office 365
• Experience with Electronic Health Records software preferred
• Physically and mentally able to perform the tasks
